4 yr old female presents to the clinic today for evaluation of rash. Accompanied by her mother. According to her mother, rash developed as a small red papule below nose and upper lip, prior to the appt today. Mother states pt may have scratched area. Since the onset of the rash, additional papules have formed that became fluid-filled vesicles for a brief amount of time, but then developed into a scab with honey-colored crusting. C/o occasional itching to the area, and thus has been scratching the area, which the mother feels has contributed to the spread of the rash. Denies any fever, reports normal activity level, and normal appetite. Has not given any medication for the rash, but did apply hydrocortisone cream 1% to the area with no apparent benefit. Denies any other symptoms
