A 42-year old female with medical diagnosis of Type 2 Diabetes Mellitus, Hypertension, Hyperlipidemia, and Obesity, BMI 38 has been referred to the RDN for outpatient Medical Nutrition Therapy services. The patient was newly diagnosed one month ago following routine lab work and reports a family history of diabetes and heart disease. Patient takes a statin, fish oil supplement, a daily multivitamin, and was started on metformin upon diagnosis. She is a single parent of 2 children works varying shifts and frequently relies on frozen meals and convenience options. She is not routinely exercising at this time.
