SOAP note example A 21 year old college junior basketball player states that during practice she felt a sharp pain in her chest. She experiences shortness of breath, chest tightness, wheezing, a rough cough. Her current pain level is 2 out of 10 at rest and 8 out of 10 pain when practicing or conditioning. The chest pain is only present when she is practicing or conditioning. She has never experienced this before getting to college. She cannot run up and down the court no more than 2 times before she is completely out of breath. She mentions that she had the flu 2 weeks ago but has completed all medications prescribed. She has been staying hydrated and eating nutritious meals three times a day for the past three days and resting but the chest pain returns when practicing or conditioning. She has a game this Saturday and she is the starting power forward. She is currently not taking any medications.
