HPI: A 55 y/o female presents with burning and cramping in chest x1 day. Describes it as 5/10 in intensity. Reports that it gives her a lot of anxiety and is accompanied by nausea. She has experienced this before; it first appeared 6 years ago when her mother passed. She reports it feels like acid or burning in her throat that leaves her throat hoarse and feeling coated. She felt some relief after her chiropractor did a hiatal hernia maneuver. PPI’s and Tums provide some relief, but not much. Stress triggers it. Symptoms: 1. Location – Burning sensation in center of chest with cramping in chest 2. Quality – Getting worse 3. Quantity or severity – 5/10 4. Timing, including onset, duration, and frequency – Since one day ago, however, it has occurred off and on throughout the years. 1 year ago, she noticed it becoming more prominent.
