Joe Hernandez is a 58-year-old man with known history of HIV diagnosis who has developed AIDS. He was provided with education on the importance of taking his ART but never quite adhered to his medication regimen. His significant other did not remain in the client’s life, and their relationship ended shortly after he revealed to her his HIV diagnosis. Since then, Mr. Hernandez has declined assistance from his daughters who visit him at least 3 times a week to make sure he is taking his meds. The youngest daughter is most often at his side, trying to get him out of the house. She has noticed his weight loss and most recently that his ankles are red, and one of them has an open sore; also, he tells her he doesn’t want to eat because the sores in his mouth are painful. Hernandez is currently admitted to the M/S unit at the local hospital with the 4th bout of pneumonia in the past 3 months. He has been advised by his attending doctor that he has developed AIDS.
