In Phase 4, you will measure and monitor the implementation of your DNP Project and will conduct regular meetings with your DNP project mentor to track progress and completion of project milestones.
For Step 1, you will review your work plan based on the Appendix I: Translation and Action Planning Tool that you previously submitted as you continue to measure and monitor the implementation of your DNP Project. You will also conduct a meeting with your DNP project mentor to assess the state of your DNP Project.

What’s Happening to This Phase?
For Phase 4, you will review your work plan based on the Appendix I Translation and Action Planning Tool and other project materials you previously submitted as you continue to measure and monitor the implementation of your DNP Project. You will also conduct a meeting with your DNP project mentor to assess the state of your DNP Project. You will address this with your DNP project mentor and share updates during your measuring and monitoring meeting. If you have no updates or changes to your action plan or implementation materials, you may not have any activities during this phase.

LEARNING OBJECTIVES

During this phase, the main objective is to review your work plan on a regular basis to determine how your DNP Project is progressing in terms of timeline.

To Prepare
• Review the DNP Project Process Guide provided in the DNP capstone resources website.
• Reflect on the current state of your DNP Project and how it is progressing in accordance with your previously developed Appendix I Translation and Action Planning Tool, Project Management Timeline
• Conduct your meeting with your DNP project mentor and identify any challenges and potential solutions with the progress of your DNP Project.
• Be sure to document and record the minutes of your meeting for this Assignment.
• If your DNP Project is not adhering to your previously submitted Appendix I Translation and Action Planning Tool, be sure to revise and update your Appendix I Translation and Action Planning Tool, following discussion with your DNP project mentor.
By Day 7
Submit your completed Minutes template.
Using the Minutes template, address the following:
• Clearly record and detail the minutes of your meeting with your DNP project mentor related to the current state of your DNP Project.
• Note: If you need to update your Appendix I Translation and Action Planning Tool, you will also submit your revised Appendix I Translation and Action Planning Tool, for this Assignment. Upon faculty advisor approval, you will share your updated Appendix I Translation and Action Planning Tool, with your DNP project mentor.
