Past surgical history includes a c-section. She takes levothyroxine and has a mirena iud. She denies dysuria and is in a monogomous relationship with her husband of 20 years. She tells you she is not spotting and does not have periods since the Mirena was placed. She reports having COVID 1 month ago. It was an uneventful course which lasted about 5 days. When she returned to work she was urged to get the COVID vaccination. She had a strong reaction to the vaccine which included muscle aches, joint pain, cola colored urine, and extreme fatigue for four days. On day 5 she is present for an evaluation and is concerned about her kidneys.
