This assignment is intended to build on the information you gathered and documented in the Individual and Family Assessment. Students will reference their chosen film character for this assignment. Students will use the Action Plan Template posted on Canvas to complete this assignment.
Students are expected to utilize the client’s needs list Individual and Family from the Assessment to describe their client’s problem in the problem statement.
You will establish two client-centered SMART goals based on your client’s identified needs. Each goal will have three corresponding SMART objectives. Each objective will have three corresponding tasks.
(The client needs list may change after you receive feedback on the Individual and Family Assessment. That’s ok.)

Each goal and the accompanying tasks must be logical, clear, and well-thought-out. The client’s goals must correspond to issues presented in the film and be based on your client’s experiences. Cragg from the movie kind of funny story
The goals must be culturally responsive. This means that the goals take your client’s age, race, gender identity, sexual orientation, ability, immigration status, dominant language, etc. into account.
You will need to use some creativity in establishing the goals as you are not in direct contact with a client for this assignment. It will be most helpful if you imagine yourself co-constructing this plan with your client.
Ideally, the challenge/problem that you will focus on must be one within your area of competence. At this stage of your studies, a social worker is not providing clinical services. For example, problems that might benefit most from intensive therapy require a referral to a clinician. Problems that might be helped via case management services, advocacy, and counseling in the areas of life skills and communication are more appropriate. Referring your client to a resource that would respond to their needs is appropriate (housing, public assistance, career readiness, education, etc.).

The assignment must be written in third person (Client, Mr. Hunting, SW, Social Worker)
Correct grammar, spelling, and punctuation are expected.
