As part of your justification and plan be sure to identify: (1) the purpose, (2) goal of the project, (3) key stakeholders, team/multi-disciplinary group members, and (4) their roles related to impacting the patient satisfaction.  Also, be sure to add a brief description of why these departments are key to the success of the project.
