:Mrs Radclifff is a married mother of one son and has a grandchild. She is unemployed.She is a christian.She stays with her husband at her house.She is at an adult day care centre during the day.The son reported that him and the husband always make sure she is taking her medications.She presented as alert and oriented x4 with confusion. Her thought process was tangential and confused.She had visual hallucinatios. She asked where the cat that was in the room had gone. Her medication was reviewed and a prescription was sent to her pharmacy as provided by the son.Follow up in two weeks.Son was requested to accompany the mother during her next visit.
