Since I am using Schizophrenia as the topic for my Chronic Condition Assignment, I’m going to be using it in our discussion this week. The OPT model, or Ottawa Patient Decision Aid Framework, is a guide for creating decision aids. These aids help patients navigate treatment choices, breaking it down into five steps, from defining the decision to getting clear on the patient’s values. It’s about getting patients involved and tailoring decisions to what matters to them (Stacey et al., 2017).
Now, the CCCR model, or Care Coordination Clinical Reasoning model, is what I would use to deal with folks facing schizophrenia. This involves teamwork, involving a multidisciplinary team – psychiatrists, nurses, social workers – all working together to take care of both the medical and psychosocial sides of care. Giving the patient a full spectrum of support (National Institute for Health and Care Excellence, 2014).
In my day-to-day at work, I’ve used the OPT model when looking into treatment options for chronic conditions like diabetes. Using decision aids based on OPT, it’s been a godsend, helping patients weigh the pros and cons and align their choices with what matters most to them.
With the CCCR model and schizophrenia, having a whole team – psychiatrists, nurses, and everyone else – ensures we’re covering all bases. It’s not just about meds; it’s about addressing the whole person.

Recent shifts in healthcare institutions underscore a growing focus on coordinating patient and family care (Karam et al., 2021). The Outcome-Oriented Patient-Centered Team (OPT) model stands out as an effective strategy, involving health providers mobilizing peers to achieve substantial patient health goals, encompassing needs identification, care prioritization, and effective communication (Rahman et al., 2023). I’ve applied this model in my past practice, collaborating with a cardiac physician to ensure comprehensive care for a patient with cardiac arrest. This ensured their appropriate management.
Another noteworthy model is the Care Coordination and Relational Coordination (CCCR) model, which aims to improve communication between healthcare providers and families (Kuiper et al., 2016). In my role as a chemo charge nurse, I implemented the CCCR model by prioritizing patient concerns and treatment plans in the Electronic Health Record (EHR), effectively reducing communication barriers and promoting swift recovery. These models serve as valuable references in my patient interactions, ensuring holistic care coordination.
