The patient is a 17-year-old female Caucasian who presents herself to the Emergency Department. Pt was accompanied by her father and grandmother. The patient has Hx of borderline personality disorder, PTSD, generalized anxiety disorder (GAD), major depressive disorder (MDD), suicidal attempts, nightmares, and eating disorders. According to the patient’s father, the patient has episodes of repeated bouts of suicidal ideation and self-injurious behavior (SIB) and recently tried to inflict injury upon herself by slashing her wrist with scissors. In addition, the patient wrote a statement on her mobile phone, in which she expressed regret for her conduct and apologized for the delay in seeking assistance. Per pt, she has nightmares related to her PTSD and she doesn’t want to talk about it at this time.
